2026 Medicare Cost Tool

Turn the federal tables into a clearer Medicare premium picture.

Enter the tax filing status and modified adjusted gross income used for your 2026 Medicare determination, then add the fixed premiums you want included. The tool estimates published Part B and Part D income-related amounts without recommending tax, investment, or retirement-account changes.

This estimates Medicare premiums. It does not provide a strategy for changing your income.

Silver Path Benefits Insurance Services can explain how published Medicare premium rules affect insurance costs. We do not advise you to sell investments, change retirement-account withdrawals, complete a Roth conversion, alter a tax filing position, or take another financial action to reach an IRMAA threshold.

Before you use the tool
  • Do not enter a Social Security number or Medicare number.
  • Use the MAGI and filing status from the tax year SSA used, or the figures shown on your notice.
  • SSA makes the actual IRMAA determination.
  • Plan-specific premiums and benefits must be verified separately.

Estimate fixed Medicare premiums and IRMAA in one place.

The result uses the 2026 federal Part B and Part D IRMAA tables. It is not a total-health-care-cost estimate and does not include care you may use during the year.

Step 1

Income information

For 2026 Medicare premiums, Social Security generally uses 2024 tax information. If 2024 information was unavailable, SSA may use 2023 information.

How would you like to enter MAGI?
Step 2

Who should be included?

IRMAA is assessed per Medicare beneficiary. A joint tax return can therefore produce an adjustment for one or both spouses depending on who has Medicare coverage.

Your 2026 estimate will appear here.

Enter MAGI, confirm filing status, add the fixed premiums you want included, then calculate.

The 2026 IRMAA table generally looks back two tax years.

For 2026 Medicare premiums, Social Security generally uses 2024 tax information, or 2023 if 2024 information is not available. The income measure is modified adjusted gross income.

01

Start with AGI

SSA-44 asks for adjusted gross income as reported on the applicable federal tax return.

02

Add tax-exempt interest

Tax-exempt interest is included in the MAGI calculation used for IRMAA. Certain special tax provisions can require additional modifications.

03

Match the filing-status table

Single-type, joint, and certain married-filing-separately situations use different 2026 thresholds.

04

SSA makes the determination

The estimator organizes published numbers. Your SSA notice and agency determination control what you actually owe.

Part D plan premiums vary by plan. The Part D amounts above are added to the applicable plan premium. Married filing separately taxpayers who lived apart for the entire tax year can fall under the individual-style table when Social Security's rules are satisfied.

A lower current income does not automatically change IRMAA, but certain SSA-listed life events can support a new determination request.

Social Security lists eight qualifying life-changing event categories for IRMAA requests. Select a category to see the general description. SSA determines whether the event, evidence, and income information meet its requirements.

SSA-listed life-changing event

Select an event

Choose one of the eight categories to review why it may matter to an IRMAA request.

What this does not mean

Selecting an event here does not establish eligibility, prove the event, or calculate a revised IRMAA. Social Security reviews the request and evidence.

SSA: Request to Lower IRMAA

Fixed premiums are only one part of Medicare spending.

This tool intentionally avoids pretending it can predict total health care costs.

Care you use

Deductibles, copays, and coinsurance.

Prescription spending

Drug-specific prices, formulary rules, and pharmacy costs.

Late-enrollment penalties

Part B and Part D penalties require separate facts and official determinations.

Plan-specific reductions

Any Medicare Advantage Part B premium reduction or other plan-specific adjustment.

Cost assistance

Extra Help, Medicare Savings Programs, Medicaid or Medi-Cal, and other subsidies.

Tax or investment effects

No retirement, securities, tax, or estate-planning recommendation is produced.

The estimator is built from federal 2026 Medicare and Social Security tables.

Educational estimator and professional-scope notice: This tool provides a general estimate using published 2026 Medicare Part B and Part D income-related monthly adjustment tables and amounts you enter. It is not an official Social Security Administration or Medicare determination, bill, tax calculation, financial plan, investment recommendation, legal opinion, or accounting advice. Actual premiums can differ because of late-enrollment penalties, Medicare Advantage premium reductions, plan-specific premiums, subsidies, billing adjustments, or other circumstances. Silver Path Benefits Insurance Services does not recommend changing investments, retirement-account withdrawals, Roth conversions, tax filing status, asset sales, or other financial transactions to alter IRMAA. Verify your actual determination with Social Security, Medicare, official notices, plan documents, and appropriate tax, legal, or financial professionals.
