Income information
For 2026 Medicare premiums, Social Security generally uses 2024 tax information. If 2024 information was unavailable, SSA may use 2023 information.
Who should be included?
IRMAA is assessed per Medicare beneficiary. A joint tax return can therefore produce an adjustment for one or both spouses depending on who has Medicare coverage.

The estimate excludes deductibles, copays, coinsurance, prescription out-of-pocket costs, late-enrollment penalties, Medicare Advantage Part B premium reductions, subsidies, and other adjustments not entered here.
The 2026 IRMAA table generally looks back two tax years.
For 2026 Medicare premiums, Social Security generally uses 2024 tax information, or 2023 if 2024 information is not available. The income measure is modified adjusted gross income.
Start with AGI
SSA-44 asks for adjusted gross income as reported on the applicable federal tax return.
Add tax-exempt interest
Tax-exempt interest is included in the MAGI calculation used for IRMAA. Certain special tax provisions can require additional modifications.
Match the filing-status table
Single-type, joint, and certain married-filing-separately situations use different 2026 thresholds.

Part D plan premiums vary by plan. The Part D amounts above are added to the applicable plan premium. Married filing separately taxpayers who lived apart for the entire tax year can fall under the individual-style table when Social Security's rules are satisfied.
A lower current income does not automatically change IRMAA, but certain SSA-listed life events can support a new determination request.
